​A alteração do estado mental e os distúrbios cognitivos aliados têm consequências devastadoras para os indivíduos infectados pelo HIV e seus cuidadores. Embora as infecções oportunistas e neoplasias relacionadas ao HIV possam também apresentar um declínio cognitivo progressivo e alterações da personalidade, elas se manifestam mais frequentemente como uma emergência neurológica aguda ou subaguda. Os pacientes também apresentam risco de acidente vascular cerebral (AVC) isquêmico, e isso deve ser considerado no quadro de deterioração neurológica aguda.

Além de diminuir a incidência de infecções oportunistas do SNC e prolongar a sobrevida, a terapia antirretroviral combinada (TAR) reduziu a prevalência da forma mais grave de comprometimento cognitivo do HIV (demência associada ao HIV [DAH]) e melhorou a qualidade de vida dos indivíduos com HIV que apresentavam problemas cognitivos.[4]Heaton RK, Clifford DB, Franklin DR Jr, et al. A comorbidade psiquiátrica é altamente prevalente em indivíduos infectados pelo HIV.
