Patient discussions

Encourage patients with chronic kidney disease (CKD) to take an active role in managing their disease. Dietary therapy such as restriction of potassium, phosphorus and salt, protein, and fluids is typically advocated for GFR category G3 to G5.​ [ Cochrane Clinical Answers logo ] ​​​

Advise patients that adherence to treatment, and optimisation of glycaemic control and blood pressure, delays progression of CKD and the need for kidney replacement therapy.

Education classes at a CKD clinic are recommended for patients with GFR category G4 CKD.

Discussion of different dialysis modalities, such as haemodialysis and peritoneal dialysis, is required to enable informed choice and care planning. Patients should be evaluated for kidney transplantation and referred to a transplant centre, if eligible.

Patients who have attended education classes and who have selected a dialysis modality can be referred for surgery for dialysis access placement.
